Semaglutide: One Receptor, Broad Evidence
The GLP-1 receptor agonist that put incretin pharmacology on the clinical map — with a head-to-head trial showing it was subsequently surpassed by its dual-agonist successor.
The short version
Semaglutide is a GLP-1 receptor agonist — a synthetic peptide that mimics the gut hormone glucagon-like peptide-1. It is FDA-approved for type 2 diabetes, chronic weight management, reducing major cardiovascular events in adults with obesity or overweight and established cardiovascular disease, and (since 2025) metabolic dysfunction-associated steatohepatitis (MASH). It comes as a once-weekly subcutaneous injection and a daily oral tablet.
In the landmark STEP 1 weight-loss trial, once-weekly semaglutide 2.4 mg produced an average -14.9% body weight change versus -2.4% with placebo at 68 weeks [4]. In SELECT, a trial of more than 17,600 adults with cardiovascular disease, it reduced major adverse cardiovascular events by 20% relative to placebo [3]. In FLOW, it reduced the risk of major kidney-disease events by 24% in people with type 2 diabetes and chronic kidney disease [2].
The honest context: semaglutide lost the head-to-head weight comparison in SURMOUNT-5, where tirzepatide outperformed it by roughly 6.5 percentage points [1]. This page summarizes what was studied; it lists no dose as advice, and none of its content is medical guidance.
What it is
Semaglutide is a 31-amino-acid synthetic peptide built on the backbone of human GLP-1, sharing approximately 94% sequence homology with the native hormone. Two substitutions confer protease resistance: at position 8, alanine is replaced by alpha-aminoisobutyric acid (Aib), which blocks cleavage by dipeptidyl peptidase-4 (DPP-4); at position 34, lysine is replaced by arginine. The remaining lysine at position 26 is then acylated with a C18 fatty di-acid side chain via a spacer, driving strong reversible binding to serum albumin. That albumin interaction shields the peptide from renal clearance and enzymatic metabolism, producing a roughly one-week circulating half-life that is the structural basis for once-weekly dosing.
For the oral formulation, semaglutide is co-formulated with sodium N-(8-[2-hydroxybenzoyl]amino)caprylate (SNAC), an absorption enhancer that briefly raises the pH of a small stomach-surface area and enables absorption before gastric acid degrades the peptide. Oral bioavailability remains low (~0.4-1%), requiring strict fasted administration [5].
How it works
Semaglutide activates the GLP-1 receptor (GLP-1R) throughout the body. At the pancreas, it enhances glucose-dependent insulin secretion from beta cells and suppresses inappropriate glucagon release from alpha cells — meaning it works only when blood sugar is elevated, which limits hypoglycemia risk when it is used alone. It slows gastric emptying, which blunts postprandial glucose spikes and contributes to nausea as a side effect.
Its weight effect is primarily central. Semaglutide crosses into hypothalamic and brainstem appetite circuits — particularly the arcuate nucleus (activating anorexigenic POMC/CART neurons and suppressing orexigenic NPY/AgRP neurons) and the area postrema — reducing both overall food intake and the intrusive background preoccupation with food that patients describe as "food noise." This central action distinguishes GLP-1 receptor agonists from simple metabolic interventions.
Beyond glucose and weight, GLP-1 receptors in the heart and kidneys mediate pleiotropic protective effects that are now established by outcome trials [2][3].
What the research shows
Head-to-head against tirzepatide. SURMOUNT-5, a 72-week phase 3b open-label trial of 751 adults with obesity and no type 2 diabetes, directly compared the maximum tolerated dose of tirzepatide against the maximum tolerated dose of semaglutide. Tirzepatide produced -20.2% mean weight loss versus -13.7% with semaglutide — a statistically significant difference of roughly 6.5 percentage points (P<0.001) [1]. This is the most recent direct comparison and places semaglutide as the benchmark the newer dual agonist surpassed.
Weight management (STEP 1). In 1,961 adults with overweight or obesity and no diabetes, once-weekly subcutaneous semaglutide 2.4 mg produced -14.9% mean body weight at 68 weeks versus -2.4% with placebo — a treatment difference of approximately 12.4 percentage points [4]. Cardiovascular outcomes (SELECT). In 17,604 adults with preexisting cardiovascular disease and overweight or obesity but no diabetes, once-weekly semaglutide 2.4 mg reduced the primary composite of cardiovascular death, nonfatal myocardial infarction, or nonfatal stroke versus placebo (HR 0.80; 95% CI 0.72-0.90; P<0.001) — a 20% relative risk reduction [3].
Kidney outcomes (FLOW). In 3,533 adults with type 2 diabetes and chronic kidney disease, once-weekly semaglutide 1.0 mg reduced the primary composite of kidney failure, ≥50% eGFR decline, or kidney- or cardiovascular-related death versus placebo (HR 0.76; 95% CI 0.66-0.88), representing a 24% lower risk [2].
Safety profile. A dedicated safety review characterizes semaglutide's risk-benefit balance as favorable in type 2 diabetes, with mostly mild-to-moderate gastrointestinal adverse effects (nausea in roughly one-third of patients), an increased risk of biliary disease (cholelithiasis), and unconfirmed signals for pancreatic and thyroid cancer owing to low incidence [5].

People using semaglutide in clinical contexts and patient communities report a consistent cluster of effects — these are compiled from published patient-experience studies and pharmacovigilance data and are labeled accordingly.
Reported benefits (anecdotal, not clinical findings): The most frequently described benefit is a dramatic reduction in food preoccupation — what reviewers consistently call a quieting of "food noise." Many describe feeling full on substantially smaller portions and, for the first time, going stretches of time without thinking about food. Weight loss follows steadily over months. People with type 2 diabetes commonly report markedly improved glucose readings. An unexpected secondary effect, noted widely in patient communities, is reduced interest in alcohol.
Reported adverse effects (anecdotal, not clinical findings): Nausea is the dominant complaint, mentioned by roughly a third of reviewers, typically peaking in the first weeks and after dose increases and easing as the body adjusts. Characteristic sulfur-smelling burps are reported frequently and described as more common than official lists acknowledge.
